Bismuth is a metal element used in medicine, particularly in treating stomach ulcers caused by Helicobacter pylori (H. pylori), a bacterium that infects the stomach lining. It’s commonly included in quadruple therapy alongside antibiotics to improve eradication success.
🔬 How Bismuth Works Against H. pylori
1. Direct Antibacterial Action
Disrupts bacterial cell walls and membranes: Bismuth interferes with the integrity of the H. pylori cell envelope, causing leakage of cellular contents and death of the bacteria. Inhibits bacterial enzymes: It targets key enzymes like urease and ATPase that H. pylori uses to survive the acidic environment of the stomach.
2. Prevents Bacterial Adhesion
Bismuth prevents H. pylori from attaching to stomach epithelial cells, which is necessary for colonization and damage.
3. Inhibits Biofilm Formation
H. pylori can form biofilms, a protective layer that shields it from antibiotics. Bismuth disrupts these biofilms, making the bacteria more vulnerable to other drugs.
4. Synergistic Effect with Antibiotics
Bismuth enhances the effect of antibiotics like metronidazole and tetracycline, especially against drug-resistant H. pylori strains. Quadruple therapy (bismuth + 2 antibiotics + proton pump inhibitor) has been found to be more effective than triple therapy.
💊 Common Bismuth-Containing Regimens
Bismuth subsalicylate (Pepto-Bismol) or bismuth subcitrate potassium is combined with: Tetracycline Metronidazole Proton Pump Inhibitor (PPI)
This is known as bismuth-based quadruple therapy, often used when initial treatment fails or resistance is suspected.
